Below is a list of insurances that the SACMIS office currently accepts. Once you have attended a seminar, our patient advocates will verify your insurance benefits within 72hrs. Please complete the insurance verification form below and return it with a copy of both sides of your insurance card to expedite the process. All patients are required to have a nutritional and psychological evaluation. We highly encourage consultation with our affiliated providers to ensure proper assessment for insurance authorization. Not all nutritionists and psychologists have specialized training for bariatric surgery.

MEDICARE

For bariatric procedures, Medicare coverage requires facilities to have Center of Excellence (COE) certification from the Surgical Review Corporation (SRC). SACMIS and Northwest Medical Center have received their certification from SRC as of April 13, 2009. Pre-surgical requirements such as diet history for insurance authorization vary widely based on whether you have a Medicare supplement plan or regular Medicare. All Medicare patients are required to have a psychological and nutritional evaluation prior to clearance for surgery at the cost of $375.00. Medicare patients are encouraged to attend a free seminar and schedule an initial consult, so that their patient advocate can explain their surgical requirements.

AHCCCS - AZ Medicaid

SACMIS does NOT contract with any AHCCCS providers for bariatric surgery and therefore cannot operate on patients that have AHCCCS as either primary coverage or as secondary coverage to private insurance. We are also unable to perform any self-pay procedures on AHCCCS patients. We encourage you to contact your plan administrator for a referral to another provider.
